Many insurances now require a referral from your primary care physician in order to see a specialist. Please confirm with your insurance whether an insurance referral to a specialist is required. As a courtesy we do try to make sure these are entered prior to any scheduled specialist appointments but it is your responsibility to make sure any insurance referrals needed are current and active.
Prescription Refill Fee:
There is a $20.00 fee for Prescriptions requested outside your follow-up visits and a $30.00 fee for all other Prescriptions.
**Scheduled Drugs: We do not routinely prescribe or refill controlled substances or chronic pain medications.**
No-Show Fee:
There is a $35 dollar charge when patient does not show for their appointment. Please click here to see the policy.
Test Results Policy:
The Physician will discuss lab results and test results with you personally at your follow up visits. If there is an urgent test result, our office will notify your sooner. You are welcome to review your results via our Patient Portal or request a copy.
